No dentist–patient relationship is created by using this Website, submitting a form, sending a message, or booking an appointment. A dentist–patient relationship is established only after we evaluate you and you complete our intake/consent process.
2) Emergencies
If you have a medical emergency, call 911 or go to the nearest emergency room. Dental emergencies outside business hours should be handled through our emergency instructions (if available) or by seeking urgent care.
3) Eligibility and Minors
This Website is intended for users who can form legally binding agreements. If you are under 18, a parent/guardian must book appointments and submit information on your behalf.
4) Appointments, Scheduling, and Requests
Online booking is a request for services and does not guarantee availability, timing, or clinical appropriateness. We may contact you to confirm, adjust, or cancel an appointment request if needed.
Clinical decisions are made in-office after evaluation. Treatment recommendations may change based on exam findings, imaging, health history, and patient goals.
5) Payments, Estimates, and Insurance
If the Website references fees, membership plans, or insurance estimates, those are estimates only and not a guarantee of coverage or final cost. Coverage depends on your plan, eligibility, medical necessity criteria, annual maximums, deductibles, and carrier rules.
You are responsible for charges not covered by insurance, including denied or reduced claims. Final charges are determined at the time of service and/or after claim processing.
6) Cancellations, No-Shows, and Late Arrivals
We require advance notice for cancellations/reschedules. Late arrivals may need to be rescheduled to protect other patients’ time and clinical quality. No-show or late cancellation fees may apply where permitted by law and disclosed to you.

By providing a phone number and/or email, you consent to receive communications from us related to:
